Warren Nursing & Rehab is seeking a compassionate Social Services Coordinator to support residents and their families, addressing psychosocial and social needs during their stay.

The role includes coordinating care planning, advocating for residents, and collaborating with nursing, therapy, and other departments to ensure person-centered care. A Bachelor’s degree in a related field is preferred.

Qualifications

• Strong communication, organization, and interpersonal skills are required.
• Bachelor’s degree in Social Work, Human Services, Healthcare Administration, Psychology, Sociology, or related field preferred.
• Experience in skilled nursing, long-term care, healthcare, or social services preferred.

Responsibilities

• Provide social services and support to residents and families.
• Complete social service assessments and maintain accurate, timely documentation.
• Assist residents with adjustment to the facility and changes in their condition or circumstances.
• Advocate for residents and address concerns, preferences, and needs.
• Participate in care planning and interdisciplinary team meetings.
• Maintain communication with residents, families, responsible parties, and facility staff.
• Assist residents and families with discharge planning and coordination of community resources.
• Provide information and referrals to appropriate community agencies and services.
• Assist with advance directives, resident rights, and other psychosocial needs.
• Respond to resident and family concerns in a professional and compassionate manner.
• Collaborate with nursing, therapy, activities, dietary, administration, and other departments to promote person-centered care.
• Participate in facility programs, meetings, and quality improvement initiatives as appropriate.

The Social Services Coordinator plays an important role in supporting residents and their families while helping ensure their psychosocial, emotional, and social needs are addressed throughout their stay.
