Job summary

Plumas District Hospital in Quincy, CA is seeking a Social Service Designee to coordinate the social service program for residents in the Skilled Nursing Facility. You will assess needs, develop individualized care plans, and refer to community resources as needed.

Ideal candidates hold a Social Service Coordinator certificate andCA CNA license with CPR preferred, and you will document on MDS sections while collaborating with multiple departments to support residents and families.

  • Requires CA CNA license within 6 months of hire.

Responsibilities

  • Assess residents' social service needs at admission and quarterly thereafter with an individualized plan.
  • Complete psychosocial sections of the MDS form and care plans.
  • Maintain progress notes on residents' social service needs.
  • Coordinate upcoming appointments with other departments.
  • Interview and assess each patient after admission to the service.

Job description

Nature and Scope

Under the direction of the Director of Nursing, the Social Service Designee is responsible for the coordination and direction of the social service program of the Skilled Nursing Facility (SNF). Identifies the medically related social and emotional needs of the resident and arranges for the meeting of these needs through individual and group social service programming and appropriate referrals to outside social service agencies, community resources and professional services.

We are located at 1065 Bucks Lake Road, Quincy, CA 95971.

Compensation

Compensation for Benefited, Full-TIme Pay Scale without a CNA

  • Base Rate: $26.68–$38.04 per hour (or annually $55,494.40 - $79,121.74)
  • Starting pay is determined using our clinical pay scales, which feature 13 steps recognizing up to 30 years of relevant experience.
  • Eligible employees may also receive shift differential and holiday pay.

Compensation for Benefited, Full-TIme Pay Scale with a CNA

  • Base Rate: $28.26–$40.29 per hour (or annually $58,780.80 - $83,807.37)
  • Starting pay is determined using our clinical pay scales, which feature 13 steps recognizing up to 30 years of relevant experience.
  • Eligible employees may also receive shift differential and holiday pay.

Qualifications

Required Education:

  • Social Service Coordinator Certificate within three (3) months of hire.
  • Obtain CA Certified Nursing Assistant license within six (6) months of hire

Preferred Education:

  • Current CPR Certification

  • Fulfills responsibility to the facility by adhering to its philosophy and general policies.
  • Is able to communicate in English both verbally and in writing.
Responsibilities
  • Assuring that the social service needs of each resident are assessed on admission and quarterly thereafter and that an individualized plan of care is developed and carried out related to his/her psychosocial concerns.
  • Completes the psychosocial sections of the MDS form in a timely manner and care plans.
  • Assures that progress notes are maintained on each resident specific to his/her social service needs that interventions address the needs and documentation reflects progress or lack of progress toward achieving the specific goal.
  • Keep all departments aware of upcoming appointments and needs required from those departments.
  • Interview and written assessment of each patient after admission to the service.
