Job summary

Care Initiatives in Kingsley, IA, is seeking a Social Services Coordinator to support residents and families with responsive, compassionate care throughout their healthcare journey. The role focuses on psychosocial assessment, discharge planning, and coordination with the care team to improve quality of life.

You will document care plans, provide counseling, and engage with community resources while upholding regulations and person-centered care across our 43 Iowa communities.

Qualifications

  • Bachelor’s degree in social work, gerontology, or related health field including sociology, special education, rehabilitation, counseling and psychology.
  • Ability to obtain a Licensed Social Worker (LSW).
  • Minimum of one year of supervised social work experience in a health care setting working directly with individuals.
  • Strong interpersonal and communication skills for effective, timely communication.
  • Commitment to empathetic, respectful, patient and discreet care.

Responsibilities

  • Assess residents’ psychosocial needs and complete social histories.
  • Complete assigned MDS sections accurately and on time.
  • Document care plans, progress, and follow-up per regulations.
  • Provide counseling and emotional support to residents and families.
  • Support residents and families with adjustment, rights, and orientation.
  • Assist with discharge planning and care transitions.
  • Coordinate referrals and liaise with community resources.
  • Partner with the care team to enhance quality of life and outcomes.
  • Plan and support resident, family, and community engagement activities.
  • Promote facility programs and services to referral partners.

What You'll Do & Key Responsibilities
