Job summary

Care Initiatives Hospice is seeking a compassionate Social Worker to support patients and families during life-limiting illness. You will assess psychosocial needs, document plans, provide counseling, and coordinate referrals to community resources across multiple Iowa locations.

We value a Bachelor's in Social Work, travel ability, and collaboration with a multidisciplinary team. Hospice experience is preferred, with competitive pay and comprehensive benefits offered.

Qualifications

• Bachelor's Degree in Social Work is required.
• Hospice or palliative care experience is preferred.
• Two years of verifiable experience as a Social Worker is preferred.
• Must have a valid driver's license and be able to travel between multiple locations in the region.

• Provide counseling and emotional support to residents and families.

Care Initiatives Hospice is looking for a Social Worker is able to commute to Iowa Falls, Hampton, Eldora and surrounding areas.

What You'll Do & Key Responsibilities:

What it takes to be a Social Worker at Care Initiatives Hospice:

• Bachelors Degree in Social Work, required.
• Hospice or palliative care experience, preferred.
• Two (2) years of verifiable experience working as a Social Worker, preferred.
• Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
