Care Initiatives Hospice – Cedar Rapids seeks a compassionate Social Worker to provide comprehensive psychosocial care for patients at home, in care communities, and hospitals. You’ll meet families where they are, delivering personalized support with dignity and respect.

Qualifications include a Master of Social Work (MSW) and an active LMSW license. Hospice experience is preferred, with ability to travel between locations within the region.

Qualifications

• Master of Social Work (MSW) degree from an accredited school of social work.
• Current, valid LMSW license required.

Responsibilities

• Assess patients’ psychosocial needs and complete social histories.
• Document care plans, progress, and follow-up per regulations.
• Provide counseling and emotional support to residents and families.
• Support patients and families with adjustment, rights, and orientation.
• Coordinate referrals and liaise with community resources.
• Partner with the care team to enhance quality of life and outcomes.
• Plan and support patient, family, and community engagement activities.
• Promote facility programs and services to referral partners.

What You’ll Do & Key Responsibilities:

What it takes to be a Social Worker at Care Initiatives Hospice:

• Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education and current, valid social work license (LMSW).
• Hospice or palliative care experience, preferred.
• Two (2) years of verifiable experience working as a Social Worker, preferred.
• Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
