Care Initiatives' Dubuque Specialty Care in Dubuque, IA is hiring a Social Services Coordinator to assess residents' psychosocial needs, complete social histories, and support transitions.

You will partner with the care team to enhance quality of life, provide counseling to residents and families, and coordinate referrals to community resources.

A bachelor's degree in social work or related field is required, with LSW eligibility and at least 1 year of supervised health-care experience.

• Minimum of one year of supervised social work experience in a health care setting working directly with individuals.
• Interpersonal and communication skills that identify the ability to positively and effectively communicate with others while completing work accurately and timely.
