Job summary

White County Rehabilitation and Nursing is seeking a Social Services Coordinator to support residents and families and assist with care coordination, discharge planning, and admissions.

In this full-time role, you will collaborate with an interdisciplinary team, address concerns, maintain accurate documentation, assist with admissions, and coordinate with hospitals and case managers to ensure smooth transitions.

Qualifications

• Experience in social services, skilled nursing, long-term care, or healthcare preferred.
• Experience with admissions or discharge planning preferred.
• CNA certification preferred.
• Bachelor's degree in Social Work or related field preferred.
• Strong communication, organizational, and interpersonal skills.
• Ability to work effectively with residents, families, and an interdisciplinary team.

Responsibilities

Pay: $18.00-$20.00 per hour
CNA: $19.50-$23.22 per hour

Schedule

• Full-Time

• Monday-Friday
