Job summary

North Bay Regional Center is seeking a Social Services Case Manager in Napa, California. You will manage service delivery for regional center clients, including those with intensive needs, ensuring that the Individual Program Plan objectives are pursued and coordinated with a team of service coordinators.

The role requires a Bachelor’s Degree in a field related to developmental disabilities; a Master’s Degree is preferred, with at least 3 years of relevant experience.

Qualifications

• Bachelor’s Degree in a field related to developmental disabilities.
• Master’s Degree preferred.
• 3 years’ experience working with individuals with developmental disability.
• Bilingual language ability (Spanish to English) is preferred.

Responsibilities

• Use person-centered planning to assist clients in using public and private resources before using regional center funds.
• Monitor vendor services and verify services are delivered as authorized.
• Evaluate the client’s Individual Program Plans with the interdisciplinary team.
• Conduct annual reassessments of clients’ developmental status.
• Conduct client assessments on an occasional basis.

Education

Bachelor’s Degree in a field related to developmental disabilities
