Job summary

Life Care Centers of America in Washington State is seeking a Social Services Assistant to support the Social Services department and address the medically related social and emotional needs of patients in accordance with applicable laws and Life Care standards.

The role involves assisting social workers, educating patients and families, coordinating with community agencies, and ensuring compassionate service in a post-acute care setting. Requires integrity, confidentiality, and teamwork.

Qualifications

• High school graduate or equivalent.
• Prior health care experience preferred.

Responsibilities

• Assist with the social services program to meet the medically related social and emotional needs of patients as well as Federal, State, corporate, and division guidelines.
• Document appropriately and timely, according to State law.
• Assist social worker(s) in supporting patients and families through education, financial planning assistance, liaison with community agencies, etc.
• Exhibit excellent customer service and a positive attitude towards patients.
• Assist in the evacuation of patients.
• Demonstrate dependable, regular attendance.
• Concentrate and use reasoning skills and good judgment.
• Communicate and function productively on an interdisciplinary team.
• Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours.
• Read, write, speak, and understand the English language.
