Job summary

You will collaborate with social workers, assist with education and financial planning assistance, document timely, and participate in an interdisciplinary team while maintaining confidentiality and high patient service standards.

Qualifications

• High school diploma or equivalent is required.
• Prior health care experience preferred.
• Ability to make independent decisions when circumstances warrant action.
• Knowledge of laws, regulations, and guidelines governing post-acute care facilities.
• Ability to read, write, speak, and understand English.

Responsibilities

• Assist the social services program to meet patients' medically related social and emotional needs and adhere to guidelines.
• Document appropriately and timely, according to State law.
• Assist social workers in supporting patients and families through education, financial planning assistance, and liaison with community agencies.
• Provide excellent customer service and maintain a positive attitude towards patients.
• Assist in the evacuation of patients.
• Demonstrate dependable, regular attendance.
• Communicate and function effectively on an interdisciplinary team.
• Follow safety and sanitation guidelines and maintain confidentiality.
