Solaris HealthCare Forest Lake in Apopka, Florida is seeking a full-time Social Services Assistant to support admissions and coordinate center services for patients’ psychosocial needs. The role emphasizes discharge planning, patient advocacy, and collaboration with families and other departments.

Qualified candidates will have at least a high school diploma and a degree in social work or psychology; crisis experience and English proficiency are preferred.

SOCIAL SERVICE ASSISTANT QUALIFICATIONS:

• High School Diploma
• Associate or Bachelor’s Degree in psychology or social work preferred (experience can substitute for preferred degree)
• 1-year experience in a senior care setting
• Experience in a long term care facility is a plus.

General Description

This position is responsible for the admission of patients to the center and coordinates center services to fulfill patients’ basic psychosocial needs as well as arranging for the discharge of the patient at the appropriate time. Serves as center ombudsman.

Qualifications/Experience/Requirements:

1. Bachelor's degree from accredited college or university, preferably BSW or related social service field
2. Experience in dealing with people in crisis is preferable
3. Knowledge of casework process, needs of geriatric patients, medical settings, working knowledge of social and health resources of community, working knowledge of funding resources such as Medicare, Medicaid, and insurances
4. Imagination and creative ability, skill in working cooperatively with other professionals, ability to organize and carry out responsibility efficiently and effectively
5. Proficiency in standard English

Responsibilities:

1. Admit patients to center, following policies and procedures outlined by State, Federal, and Company guidelines.
2. Establish and maintain a waiting list, ensuring that applicable State, Federal and Company policies are followed.
3. Participate in room assignments, ensuring that patients, families and all departments are informed.
4. Ensure that all records required to be filled out are done so completely, legibly, and in a timely fashion.
5. Be able to prepare for and participate in multidisciplinary Patient Care Planning Conferences.
6. Act as Ombudsman, informing patient and family of patient’s rights and privileges.
7. Keep accurate records of complaints, and report complaints and suggested remedies to the administrator.
8. Perform case work with patients and families, consisting of basic counseling, arranging for transportation, encouraging family participation, and others as necessary to meet patients’ needs.
9. Upon admission and throughout stay, coordinate information which will enable patient to be discharged at medically appropriate time.
10. Provide information to other facilities with whom there is a Transfer Agreement on discharge.
11. Participate in Utilization Review meetings.
12. Be aware of community resources, and be able to refer appropriate patients to those resources.
13. Develop programs for presentation to various groups regarding nursing home services and/or other marketing related aspects as designated by administrator.
14. Participate in continuing education programs available, with consent of administrator.
15. Participate in center's quality assurance program.
