Qualifications

• 1 year of social services or social work experience in a healthcare setting.
• SNF experience preferred.
• Medicaid in LTC or SNF settings preferred.
• Knowledge of Code Statuses preferred.
• PCC (PointClickCare) preferred.
• Strong communication and customer service skills, especially with elderly populations.
• Currently pursuing a degree in Social Work, Human Services, or a related field preferred.

Responsibilities

• Interview residents and families to gather social histories and identify needs.
• Assist with discharge planning and coordinate community resources.
• Support development and implementation of individualized social service plans.
• Participate in resident assessments, care planning meetings, and programs.
• Provide compassionate support and customer service to residents and families.
• Maintain accurate documentation per facility policies and regulatory requirements.
• Report occupational exposures per facility policy and procedures.
• Participate in emergency drills and follow the facility’s emergency plan.
• Stay current on industry best practices and apply them to social services programs.
• Collaborate with the interdisciplinary team to promote quality care.
