Job summary

Avante Group in Ocala, FL is seeking a Social Services Assistant to support the Social Services department in providing medically related social services for residents in our Avante at Ocala Skilled Nursing and Rehabilitation Center.

You will assist with discharge planning, PASRR paperwork, room changes, Medicaid applications, and compliance audits. The role includes coordinating with Activities staff and handling general office tasks to ensure smooth operations.

Qualifications

• Must be able to read, write and communicate in English.
• Must be computer proficient with applicable programs such as Microsoft Word, Excel, Outlook, etc.
• High School Diploma required.

Responsibilities

• Assist with discharge planning and arrangements for home health services.
• Assist with PASRR Level II paperwork and ensure timely completion.
• Handle routine social services office duties (filing, faxing, calls, mailing).
• Coordinate room changes and document in the medical record within 48 hours.
• Assist with Medicaid ICP applications and social services audits for compliance.
• Manage grievance process, investigate issues, and report to Administrator and Social Services Director.
