The Lodge Health and Rehabilitation Center in Ocala, FL is hiring a Social Services Assistant. This role supports the Social Services Director in addressing residents' psychosocial and social needs, handling admissions, discharge planning, documentation, and family communication.

You will work with residents, families, nursing staff, and the interdisciplinary team to promote dignity and quality of life, while ensuring HIPAA-compliant confidentiality.

Qualifikationen

• High school diploma or GED required.
• Experience in a skilled nursing facility or social services setting preferred.
• Familiarity with discharge planning and resident rights is a plus.

Aufgaben

• Assist the Social Services Director with daily operations and resident services.
• Support residents and families during admission, adjustment, and discharge.
• Identify residents' psychosocial needs and communicate concerns to the team.
• Participate in interdisciplinary care plan meetings and update resident information.
• Coordinate discharge planning including transportation and community resources.
• Maintain timely documentation in residents' medical records per policies.
