Job summary

The Avante Group's Avante at Ocala Skilled Nursing and Rehabilitation Center is seeking a Social Services Assistant to support the Social Services department with medically related social services.

Ideal applicants will have English proficiency, computer skills, a high school diploma, and at least 3 years of related experience; long-term care experience is preferred. Join a team dedicated to high-quality resident care.

Qualifications

• Must be able to read, write and communicate in English.
• Must be computer proficient with applicable programs (Microsoft Word, Excel, Outlook, Giftwrap).
• High School Diploma required.

Responsibilities

• Assist with discharge planning and home health service arrangements.
• Assist with PASRR Level II paperwork and ensure timely completion.
• Perform general social services office duties (filing, faxing, phone calls, mailing).
• Assist with room changes and document updates in the medical record.
• Coordinate with Activities staff to obtain personal items and adaptive equipment.
• Assist with Medicaid applications and ICP Medicaid process.
• Complete 3-day Medicare letters in absence of Social Services Director after training.
