Job summary

Genesis in Mount Olive, NC, seeks a Social Services Assistant to join a compassionate, patient‑centered team. The role supports patients and families, coordinates resources, and helps with transfers within the nursing center to ensure smooth transitions.

You will participate in care decisions under a licensed social worker or consultant, in a respected long‑term care setting. Requires an Associate degree in human services and at least two years in long‑term care, plus state certification as

• Certification required by state regulations must be possessed.

Responsibilities

• Collaborate with patients/residents and families to provide support and resource information.
• Respond to issues identified by patients/residents and their families.
• Facilitate patient/resident transfer within the nursing center and document accordingly.
• Participate in health care decision-making under direction of a social worker or consultant.

Responsibilities

• Collaborate with patients/residents and their families to provide support and resource information.
• Respond to issues identified by patients/residents and their families.
• Facilitate patient/resident transfer within the nursing center to ensure a seamless transition and patient/resident adjustment and document accordingly.
• Participate in the health care decision-making process within the nursing center under direction of a qualified social worker and/or qualified consultant.

Qualifications

• Associate degree in human services with a minimum of two years' experience in long-term care is required.
• Must possess any certification required by state regulations.
