Job Details

Job Title: Social Service Assistant
Location: Puyallup, WA 98372
Salary Range: $16.66 – $24.00 hourly
FLSA Status: Non-Exempt

ESSENTIAL JOB FUNCTIONS
  • Assist with admission process; facility tours, inquiries, interviewing prospective residents, families or responsible parties
  • Make arrangements/appointments for personal care issues/items such as hair care, transportation to appointments, and arrangements to obtain requested adaptive equipment, clothing and personal items
  • Participate in quality assurance programs, as directed
  • Maintain contact with family members and attending physicians for invitations to care plan meetings and/or other appropriate communication as requested
  • Document all contact and referrals on social service log
  • Assist in resolution of grievances and theft and loss issues as requested
  • Assist residents/families in completing various forms and applications, including advance directives, Medi‑Cal applications, voter registration, etc.
  • Participate in Behavior Management programs, as directed
  • Attend facility in‑service and staff meetings, as directed
JOB FUNCTIONS

  • Strictly adheres to the corporate integrity agreement and standard of compliance
  • Keeps current knowledge of county, state and federal laws and regulations that apply to social services in long‑term care
  • Communicates effectively with residents, families, staff, community and government agencies in written and/or oral form
  • Understands and follows written and verbal directions, and acts as a role model by protecting and promoting residents’ rights
  • Performs duties in accordance with the facility mission and philosophy
  • Is familiar with emergency and disaster procedures, can locate nearest exit and respond to written or oral instruction in case of emergency
  • Has sufficient mobility and strength to move freely through the building, and to assist, transfer or otherwise move residents in case of emergency
  • Respects co‑workers, complies with attendance, punctuality and dress code policies, and reports for duty on all shifts, including weekends and holidays
  • Complies with workplace safety policies and procedures
  • Prioritizes tasks and completes duties within allotted time, responding to change productively and handling additional tasks as assigned
  • Maintains ability to carry out essential functions with or without reasonable accommodation, ensuring no substantial harm to health and safety of self or others
  • Performs other duties as assigned
PHYSICAL CAPACITIES
  • Must be able to move intermittently throughout the workday
  • Must have functioning vision and hearing, or appropriate prosthetics, to meet position requirements
  • Must meet general health requirements, including annual TB screening and physical examination
  • Must be able to sit for extended periods of time
  • Must be able to lift and carry up to 25 pounds
  • Must cope with the mental and emotional stress of the position
ENVIRONMENTAL CONDITIONS

Primarily indoors, normal temperatures, occasional noise, fumes/odors, chemical exposure, and potential exposure to bloodborne pathogens. The role involves exposure to the stresses associated with an intimate, 24‑hour skilled care environment delivering care and services to disabled and cognitively impaired residents in an aging population.

QUALIFICATIONS/REQUIREMENTS
  • Education: High School graduate or equivalent
  • License: None required
  • Work Experience:
    • Preferred: Classes or experience in social work or related field
    • SSD Certificate enrollment within 6 months of employment
  • Language Skills: Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals
  • Mathematical Skills: Ability to perform operations using units of weight and volume; read, analyze and interpret scientific and technical information; communicate findings verbally in English
  • Communication Skills: Exceptional customer service and empathetic communication; effectively communicate with patients, families, responsible parties, staff and outside resources and agencies
