Genesis in Lebanon, PA is seeking a Social Services professional to collaborate with patients, residents and families to provide support and resources. The role focuses on addressing concerns identified by patients and families, coordinating transfers within the nursing center, and documenting accordingly.

The position requires an Associate degree in human services with at least two years in long-term care, plus any state-mandated certifications.

Qualifications

• Associate degree in human services is required; two years' experience in long-term care is also required.
• Certifications required by state regulations must be possessed.

Responsibilities

• Collaborate with patients/residents and their families to provide support and resource information.
• Respond to issues identified by patients/residents and their families.
• Facilitate patient/resident transfer within the nursing center to ensure seamless transition and document accordingly.
