Job summary

Diversicare Healthcare Services, LLC in Montgomery, AL seeks a Social Services Coordinator to support admissions and Medicaid processes, assist residents and families, and coordinate post-discharge planning.

The role emphasizes compassion, strong communication, and teamwork within a post-acute care setting, with duties spanning eligibility interviews, ICP applications, documentation, and collaboration with the Social Services Director and facility staff.

Qualifications

• High School diploma or equivalent; Bachelor's in Social Work preferred.
• One year of supervised social work experience in a health care setting.
• Experience with Medicaid ICP qualifications and applications preferred.
• Ability to work with a variety of people in different settings.

Responsibilities

• Assist in pre-admission and admission process; provide tours and paperwork as needed.
• Interview clients and families to determine Medicaid eligibility.
• Assist families in applying for ICP approval; attend annual reviews.
• Assist facility bookkeeper with Medicaid status reporting.
• Assist Social Services Director with interviewing residents and families for past/present histories.
• Document progress and update care plans at interdisciplinary conferences.
• Assist residents and families with adjustment to admission to facility.
• Help plan post-discharge services and follow-up care.
• Maintain effective communication with residents, families, health care professionals and staff.

Responsibilities

• Assist in pre-admission and admission process with customers, as needed, as back-up to Admissions Department personnel. Duties to include providing tours, signing of paperwork, etc.
• Interview clients and families to determine financial status and need for Medicaid assistance.
• Assist families in applying for clients’ Medicaid (ICP) approval. Attend annual review for Medicaid eligibility if families unable to attend.
• Assist facility bookkeeper in tracking and reporting status of Medicaid Applications/Approvals.
• Assist Social Services Director, as needed, with interviewing residents, families to obtain relevant past and present social histories, including past behaviors, adjustments, and lifestyles.
• Documents progress and updates plans of care at interdisciplinary conference, as needed.
• Assist residents and families with adjustment to admission to facility by providing one-on-one counseling and/or group discussions.
• Help Social Services Directory with planning of post discharge services and follow-up care.
• Maintain effective communication with residents, families, health care professionals and facility staff.
• Assist residents and families in any situation, as needed.
• This person may have access to protected health information in the patient’s financial and medical records to conduct job duties.

Qualifications

• Minimum of High School diploma or equivalent. Bachelor’s Degree in Social Work or Human Services is preferred.
• One year of supervised social work experience in a health care setting, working directly with individuals.
• Experience or knowledge of Institutional Medicaid Program (ICP) qualifications and application process preferred.
• Ability to work with a variety of people in situations varying from community and health care settings.
• Able to make decisions concerning procedures based on evaluations and interpretations of clients’ case histories
