Job summary

Diversicare is seeking a Social Worker to support pre-admission and admission processes, interview and assess residents and families, and coordinate care plans and services within our nursing and post-acute facilities. The role emphasizes advocacy, discharge planning and interdepartmental collaboration.

Qualifications include a Bachelor’s or Master’s in Social Work, state license, and at least one year of supervised social work in a healthcare setting.

  • Bachelor’s or Master’s Degree in Social Work.
  • State license is required.
  • One year of supervised social work experience in a health care setting.
  • Experience in healthcare; nursing homes/hospice/home health/hospital preferred.
  • Ability to work with a variety of people in different situations.
  • Ability to make decisions based on evaluations and case histories.

Responsibilities

  • Assist in pre-admission and admission process with customers; aid clients in adjusting to nursing home placement.
  • Interview residents and families to obtain relevant past and present adjustments, behavior and lifestyle.
  • Document progress and update care plans at interdisciplinary care conferences.
  • Arrange transportation, appointments, and coordinate between departments, residents’ families and outside agencies.
  • Assist in placement through counseling and group work.
  • Develop discharge plans when clients are admitted.
  • Arrange for post-discharge services and follow-up care.
  • Assess residents’ emotional, financial, spiritual, educational and social needs; coordinate with facility education/training as needed.
  • Advise residents and families on rights and advocate on their behalf.
  • Organize Family Counc meetings and maintain minutes; ensure open communication with all stakeholders.

Responsibilities


  • Assists in pre-admission and admission process with customers; aids clients in adjusting to nursing home placement.

  • Interviews residents and families to obtain relevant past and present adjustments, behavior and lifestyle.

  • Documents progress and updates plans of care at the interdisciplinary care conference.

  • Arranges transportation, makes appointments and acts as a liaison between all departments, residents’ families and outside agencies.

  • Assists in handling and accepting placement through one-on-one counseling and group work.

  • Ascertains potential and develops discharge plans when clients areadmitted.

  • Arranges for post discharge services and follow-up care.

  • Assess emotional, financial, spiritual, educational and social needs of residents; ensures those needs are Manager, Education & Training by the facility or through the use of outside agencies.

  • Acts as resident/family advocate and ensure the resident is knowledgeablein and exercises his/her rights.

  • Responsible for organizing and conduction of Family Counsel Meetings and maintaining minutes of the meeting.

  • Maintains effective communication with residents, families, healthcare professionals and facility staff.


Qualifications


  • Bachelor’s or Master’s Degree in Social Work

  • License with State

  • One year of supervised social work experiencein a health care setting, working directly with patients and families.

  • Experience in working in healthcare setting – preference in nursing home, hospice, home health, hospital.

  • Ability to work with a variety of people in situations varying from community and nursing home planning.

  • Able to make decisions concerning procedures based on evaluations and interpretations of case histories.
