Job summary

Community Extended Care Hospital of Montclair is seeking a compassionate Social Services Assistant to support residents and families in a skilled nursing setting. You will assist with admissions, adjustment to long‑term care, discharge planning, and access to community resources while upholding residents' rights and dignity.

You will help with assessments, documentation, and care plan participation, ensuring HIPAA compliance and coordinated referrals in a fast-paced environment.

Qualifications

  • Minimum one year of experience in healthcare, social services, customer service, or related field.
  • Strong communication and interpersonal skills.
  • Excellent organizational and time management abilities.
  • Basic computer proficiency and HIPAA confidentiality.

Responsibilities

  • Assist with resident assessments and psychosocial documentation.
  • Coordinate admissions, transfers, and discharge planning.
  • Provide emotional support and resource information to residents and families.
  • Complete social service documentation per state and federal regulations.
  • Refer residents to community agencies and outside services.
  • Participate in interdisciplinary care plan meetings.
  • Advocate for resident rights and assist with grievances.
  • Maintain confidential records in HIPAA-compliant manner.
  • Support adjustment to facility and identify psychosocial needs.
  • Assist with IDs, insurance, and community benefits when appropriate.

Social Services Assistant

Location: Community Extended Care Hospital of Montclair, Montclair CA

Employment Type: Full-Time

Position Summary

Community Extended Care Hospital of Montclair is seeking a compassionate, organized, and motivated Social Services Assistant to support our Social Services Department in providing quality psychosocial services to residents in our skilled nursing facility. Under the direction of the Social Services Director, the Social Services Assistant helps residents and families navigate the admission process, adjustment to long-term care, discharge planning, and access to community resources while promoting residents' rights, dignity, and quality of life.

Essential Duties and Responsibilities
  • Assist the Social Services Director with resident assessments and psychosocial documentation.

  • Help coordinate admissions, transfers, and discharge planning.

  • Meet with residents and families to provide emotional support and resource information.

  • Assist with completion of required social service documentation in accordance with state and federal regulations.

  • Coordinate appointments and referrals to community agencies and outside services.

  • Participate and manage interdisciplinary care plan meetings with residents and families.

  • Advocate for resident rights and assist with grievance resolution.

  • Help coordinate resident and family meetings as needed.

  • Maintain confidential resident records in compliance with HIPAA.

  • Support residents with adjustment to the facility and identify psychosocial needs.

  • Assist with obtaining identification documents, insurance information, and community benefits when appropriate.

  • Participate in quality improvement initiatives and regulatory surveys.

  • Perform other duties as assigned by the Social Services Director or Administrator.

Qualifications
Required
  • High school diploma or equivalent.

  • Minimum one (1) year of experience in healthcare, social services, customer service, or a related field.

  • Strong communication and interpersonal skills.

  • Excellent organizational and time management abilities.

  • Basic computer proficiency, including Microsoft Office and electronic medical records.

  • Ability to maintain confidentiality and exercise sound judgment.

Preferred
  • Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field.

  • Experience working in a Skilled Nursing Facility (SNF), rehabilitation center, or long-term care setting.

  • Bilingual (English/Korean/Spanish) is a plus.

  • Ability to communicate effectively with residents, families, physicians, and interdisciplinary team members.

Physical Requirements
  • Ability to sit, stand, walk, bend, and occasionally lift up to 25 pounds.

  • Ability to move throughout the facility during the workday.

  • Ability to use office equipment and computers.
