Job summary

Regal Heights Rehabilitation & Nursing Center is seeking an experienced Social Worker to join our caregiving team in New York. The role focuses on advancing residents’ psychosocial well-being, coordinating services, and ensuring smooth admissions and transfers while supporting families and staff.

The ideal candidate has Nursing Home or Hospital experience and holds a BSW or MSW degree, with bilingual English/Spanish being a plus. We offer competitive benefits and a collaborative work environment.

Qualifications

• Nursing Home or Hospital experience required (Internship acceptable).
• BSW or MSW in related field required.
• Bi-lingual English/Spanish preferred but not required.

Responsibilities

• Maintaining and/or improving resident’s ability to control daily physical needs and mental/psychosocial needs.
• Initiating appropriate interventions for residents experiencing mental or behavioral health issues.
• Identifying individual social and emotional needs of residents.
• Participate in planning admission, community re-entry, or transfer to another facility.
• Collaborate with the finance team to educate residents and ensure proper documentation during stay.
• Assist residents with discharge planning and arrange DME or home care for community return.

Social Worker Duties and Responsibilities:

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Initiating appropriate interventions for residents experiencing mental or behavioral health issues
• Identifying individual social and emotional needs
• Participate in the planning of the resident’s admission, community re-entry, or transfer to another facility
• Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during his or her stay
• Work with the resident on discharge and get necessary DME or home care in place so the resident is safe when he or she returns to the community
