Job summary

Avalon Health Care Group is seeking an experienced Social Services Assistant to join our Honolulu facility on a full-time basis. You will work with the Social Services Director/Coordinator to assess and address residents’ social, emotional, and psychological needs and help plan care and discharge when appropriate.

The position offers a starting wage of $25.00 per hour and a Monday–Friday schedule (8:00 am–4:30 pm).

Qualifications

• Bachelor’s Degree in Social Work or related field required.
• Healthcare experience preferred.
• Ability to read, write and follow oral and written directions in English.
• Ability to relate positively to residents and families.

Responsibilities

• Develop a comprehensive social history assessment, a psycho-social assessment, discharge plan, and complete the MDS and Care Plan for new admits. Orient and assist new residents in adjusting to the facility.
• Assist residents and families with adjustment to changes, including room changes.
• Assist families with the engraving and/or marking of residents’ personal items.
• Review residents’ rights with residents and staff, and attend to various personal needs of the residents. Assist in the development of a volunteer program and assist in facilitating family group meetings.
• Document progress notes, which relate to each resident’s care plan, when necessary and within policy time-frame.

The Social Services Assistant works with the Social Services Director/Coordinator in identifying and providing for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during their stay at the facility and to assist in the planning for their discharge.

$25.00 /hr

Placement within the wage range for this position is determined by years of experience, qualifications, and demonstrated ability to perform the essential functions.
