Job summary

Complete Care at Southpointe in Greenfield, WI is hiring a Part-Time Social Services Assistant to support residents with psychosocial and emotional needs.

You will complete social histories and psychosocial assessments, collaborate with an interdisciplinary team, uphold residents' rights, address abuse as required by law, and perform other duties as assigned.

Qualifications

• Must have long term care experience or case management.
• Ability to work independently or in a group.
• Strong rapport with residents and peers.
• Experience working with the elderly preferred.

Responsibilities

• Identify residents' psychosocial, mental, and emotional needs.
• Complete Social History and Psychosocial assessment of residents.
• Work with interdisciplinary team to promote and protect residents' rights.
• Prevent and address resident abuse as mandated by law and licensure.
• Other duties as assigned by Director of Social Services and Administrator.
