Job summary

Royale Gardens Health & Rehabilitation Center in Grants Pass, OR is hiring a Social Services Assistant (SSA) to support residents as they adjust, thrive, and find purpose during their stay. The role is part-time with weekend availability, offering meaningful daily impact.

You’ll document progress, develop social histories and discharge plans, and collaborate with families and the care team while upholding residents’ rights.

Qualifications

• Bachelor’s degree in Social Work or related field is preferred.
• Healthcare experience highly preferred.
• Ability to connect positively with residents, families, and team members.
• Must meet applicable State and Federal requirements.
• Comfortable using basic computer systems and documentation tools.

Responsibilities

• Help residents adjust to their new environment with compassion and care.
• Develop social histories, psychosocial assessments, and discharge plans.
• Support residents and families during transitions and emotional challenges.
• Document progress notes and contribute to care plans.
• Foster a positive environment through volunteer programs and family group meetings.
• Uphold residents’ rights and ensure personalized attention to individual needs.
