Job summary

Royale Gardens Health & Rehabilitation Center in Grants Pass, OR is hiring a Social Services Assistant (SSA) on a part-time, weekend-availability basis. You’ll support residents’ adjustment, develop psychosocial assessments, and contribute to discharge planning with compassion.

Qualifications

• Bachelor’s degree in Social Work or related field (preferred).
• Healthcare experience is highly preferred.
• Ability to connect with residents, families and team members.

Responsibilities

• Help residents adjust to their new environment with care.
• Develop social histories, psychosocial assessments, and discharge plans.
• Support residents and families during transitions and emotional challenges.
• Document progress notes and contribute to care plans.
• Foster a positive environment through volunteer programs and family group meetings.
• Uphold residents’ rights and ensure personalized attention.
