Talentify in Chambersburg, PA is seeking a compassionate social services professional to support residents and families in our nursing center, coordinating information and resources and responding to questions. Collaborate with the care team to coordinate transfers, participate in health care decisions under guidance, and document care plans and outcomes.

Requires an associate degree in human services and two years in long-term care, plus any state certifications.

Qualifications

• Associate degree in human services with a minimum of two years' experience in long-term care is required.
• Must possess any certification required by state regulations.

USD $25.00 - USD $30.00 /Hr.
