Job summary

Empire Care Centers at Anderson Mill Center for Nursing and Healing seeks a Social Services Assistant to provide counseling, coordinate services, and support residents and families in a compassionate, rights-respecting environment.

The ideal candidate holds a Bachelor’s degree in Social Work or higher, with prior long-term care experience and strong communication skills to advocate for residents and facilitate discharge planning.

Qualifications

• Bachelor’s degree in Social Work or higher is required.
• Prior experience in a long-term care setting is preferred.
• Strong communication and interpersonal skills are essential.

Responsibilities

• Develop and execute a comprehensive social services program, offering counseling and support services to residents and families
• Assess and address residents' medical-related social needs
• Coordinate family meetings and facilitate ongoing discussions between residents, families, and the treatment team
• Advocate for residents' rights while liaising between all parties involved
• Manage discharge planning efficiently
• Ensure residents and families receive top-notch service in a compassionate atmosphere that respects individual needs and rights
