Qualifications

Independence Rehab and Nursing is seeking a highly skilled and compassionate Social Service Director to join our team. The successful candidate will be responsible for providing a comprehensive social services program that addresses the physical, psychological, and social needs of our residents

Responsibilities

• Provide resident-centered care that recognizes each resident's physical, emotional, and spiritual needs

• Develop and implement social services programs that promote resident engagement, socialization, and community integration

• Conduct resident evaluations and assessments to identify needs and develop individualized care plans

• Collaborate with our interdisciplinary team to develop and implement care plans that promote resident well-being and independence

• Provide informal counseling and supportive services to residents and their families

• Establish and maintain relationships with community resources and services to promote resident access to outside services

• Manage referrals to outside services, including medical specialists, podiatrists, and psychological services

• Maintain accurate and up-to-date documentation of resident records and care plans

• Participate in Quality Improvement initiatives to ensure high-quality care

Requirements

• Master's degree in Social Work, Psychology, or a related field

• 2+ years of experience in social services or a related field

• Strong communication and interpersonal skills

• Ability to work effectively with diverse populations and interdisciplinary teams

• Familiarity with state and federal regulations and guidelines related to social services and long-term care
