Job summary

Christian Care Communities in Midway, KY seeks a Social Services Director to coordinate residents’ stays in the long-term care facility, act as the primary contact for residents and families, and oversee social work plans of care while coordinating with care conferences and outside services.

The role requires a Bachelor’s in Social Work or equivalent and 2+ years of hands-on coordination in acute/post-acute settings. Strong communication and advocacy are essential.

• Strong coordination and counseling skills to support residents and families.

Responsibilities

• Develops, manages and implements social service plans of care for each resident or patient, identifying problems, needs and goals.
• Assists residents in processing forms to obtain outside services (Medicaid, Medicare, Social Security).
• Schedules care conferences with MDS Coordinator for Medicare, Medicaid and managed care residents.
• Identifies and obtains resources from community social, health and welfare agencies to meet resident needs; schedules appointments.
• Provides informal counseling to uncover problems affecting residents’ socialization and participation in activities.
• Provides residents opportunities to voice problems and communicates grievances to the administrator.
• Maintains up-to-date evaluation documentation on residents’ activities in compliance with state/federal requirements.
• Contacts family to improve understanding of residents’ issues and maintain communication.

Job description

The Social Services Dir (SSD) is responsible for coordination of the resident’s stay in the long-term care facility. The

SSD will act as the main point of contact for the resident and family to answer questions, coordinate care

conferences, schedule appointments, arrange transportation, coordinate ancillary services, complete

assessments, and is the administrative liaison for issues dealing with APS, guardianship, and home health

coordination.

POSITION REQUIREMENTS:

Required Education : Bachelor’s Degree in Social Work or equivalent human services field

Required Experience : 2 years’ experience in an acute or post-acute setting with daily patient/resident/family interaction assisting with the coordination of care and services

Primary Responsibilities

1. Develops, manages and implements preliminary and comprehensive written social service plans of care for each resident or patient, identifying problems and needs, as well as goals of the resident or patient. Develops a comprehensive rehabilitative plan, which will be successful and practical.
2. Assists residents or responsible parties in processing forms or applications in the effort to obtain outside services such as Medicaid, Medicare, Social Security or any other service to which the resident may be entitled. Works closely with the MDS Coordinator to schedule care conferences within prescribed time frames for Medicare, Medicaid, and managed care residents.
3. Identifies the need for, and obtains resources from, community social, health and welfare agencies to meet the needs of the resident. Schedules appointments as needed.
4. Provides informal counseling to uncover any problems which might be interfering with the residents’ socialization and participation in home activities.
5. Provides residents with opportunities to make vocal their problems. Finds solutions to issues concerning the resident, family and facility. Communicates all grievances to the administrator.
6. Maintains up-to-date evaluation documentation on each resident’s activities at the facility in compliance with all state and federal requirements
7. Contacts family to provide a better understanding of the residents’ issues and affords the relatives a channel of communication with the facility. Establishes contact with family members during relative visitation at the facility or by telephone contact.
