Citadel Healthcare Facilities in Skokie, IL is seeking a Director of Social Services to lead and oversee social services within the rehabilitation program, ensuring regulatory compliance and high-quality resident support.
You will plan, develop, and direct social service programs, supervise staff, coordinate with other departments, and guide families through Medicare/Medicaid options while maintaining a patient-centered approach and efficient workflows.
Skokie Meadows is seeking a Director of Social Services to join their team! The Director of Social Services is responsible for ensuring social services are provided to patients throughout the rehabilitation program, either directly or by managing others in the department.
Reviews daily work assignments to ensure they meet regulatory compliance and provide consistent staffing whenever possible.
Plans, develops, organizes, implements, evaluates and directs the social service programs of the facility
Interviews residents or family members as necessary to obtain social history
Provides information to resident/families as to Medicare/Medicaid and other financial assistance programs available to the resident
Refers resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident
Assists in standardizing the methods in which work will be accomplished; collaborates with staff and other departments to develop workflow efficiencies and streamline business practices to reduce costs and eliminate waste
Develops and maintains a community and social services referral file of agencies and organizations that provide assistance to residents
Interprets the department’s policies and procedures to employees, residents, visitors, government agencies, etc.
Keeps abreast of current federal and state regulations as well as professional standards and makes recommendations in changes in facility policies and procedures to the Administrator
All other duties as assigned
Proven strong management skills, including delegating responsibility to subordinates, performance management, conflict management, and leading through others.
Must have good financial and management skills.
Ability to plan and adapt policies and procedures relating to social services
Strong organizational skills.
Ability to read, write, speak, and understand English.
Demonstrates basic computer knowledge and ability with an aptitude to learn company applications; proficient with Microsoft Office Suite.
Bachelor’s degree in social work or a human services field including sociology, gerontology, special education, rehabilitation counseling, psychology, etc. required
Master of Social work (MSW) or related field preferred
Possess a current license to practice in the State as a Social Services Professional.
Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required.
Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required.
