Job summary

O'Neill Healthcare North Olmsted is seeking a Social Service Designee/Admissions Coordinator to join our team in North Olmsted, OH. This regular full-time role involves providing social services to residents and families, coordinating admissions, and collaborating with hospital case managers and staff.

The position requires strong communication, documentation, and computer skills, with opportunities for involvement in social events and a commitment to resident well-being within a family-owned

Qualifications

  • Provide initial and ongoing social service to resident, families, and staff through room visits, staff meetings, and other interactions throughout the facility.
  • Perform all duties involved in resident advocacy.
  • Ensure the resident's psychosocial needs are being met.
  • Provide a continuing assessment of the resident care plan by direct contact with family members on a regular basis and maintaining a friendly, supportive relationship (1 to 1, family meetings). Notify the family/responsible party of scheduled care conferences, encourage them to remain involved in the care and life of a resident, and inform them of any change in the resident's psychosocial well-being.
  • Maintain proper documentation and perform general duties.
  • Conduct MDS's and assessments as needed.
  • Assist with planning the resident's discharge.
  • In charge of managed care updates for skilled residents.
  • Works as the primary contact and representative of the facility with Hospital Case Managers, Social Workers, Physicians, Liaisons, potential residents, and their families, before and during the admission process.
  • Exercises discretion and independent judgment in the facility's decision-making process of accepting or denying all referrals for admission into the facility.
  • Facilitate and coordinate daily with the Director of Nursing, providing relevant medical information regarding potential admissions from hospitals, family members, and responsible parties on all patient admissions.
  • Assist the resident and/or families in making appropriate admission decisions for rehabilitation by providing pertinent information and tours as needed.
  • Respond to all phone, email, and walk-in tours and record all inquiries about the facility.
  • Attend daily department head meetings, updating daily census and admissions.
  • Gather admission information and complete monthly reports.
  • Must project a positive attitude and promote the facility and its staff/residents in a positive manner.
  • Excellent computer skills.
  • Attend social events and conferences as deemed necessary by the Administrator.
