Job summary

New Horizons of the Treasure Coast and Okeechobee in Florida is seeking a caring SOAR Specialist for the Adult Case Management department to provide comprehensive direct support services for adults with serious mental illness. Qualifications

  • Associates degree from an accredited college or university and 3 years’ experience working with adults who have serious and persistent mental illness.
  • Bachelor's degree in a human service-related field with one year of experience working with adults experiencing serious mental illness; four years may substitute for a degree
  • Certified
  • Valid Florida Driver's License with no more than 6 points in the past 5 years
  • Must pass Level 2 Background through DCF and AHCA Clearinghouse

Responsibilities

  • Interview clients to gather information to complete SSI/SSDI applications
  • Ensures all SOAR Medical Summary Reports for client applications are complete.
  • Accompanies clients to appointments at the Social Security Administration and maintains communication with SSA and Disability Determination (DD)
  • Review cases with Local SOAR Lead. Attend staffing with SOAR lead and SEFBHN
  • Complete SOAR medical summary report for individuals application utilizing internal/external medical reports, obtain current psychiatric Dr, APRN or NHTC medical director signature for summary report.
  • Monitors progress of benefit process and provides assistance as required
  • Conducts case management activities for assigned clients to include assessment clients to include assessment of strengths and needs, service planning, service brokering, service and care monitoring, and client advocacy
  • Communicates essential information regarding client care all members of the client's treatment team, both within and outside the NHTC continuum of care
  • Provide crisis intervention services as required

New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast (and beyond), serving 15,000 children and adults annually through inpatient crisis services, 24-hour help line and mobile response team, and outpatient programs conveniently located across Indian River, Martin, Okeechobee and St. Lucie counties. In addition, New Horizons assists 22,000 students in area schools, and we work closely with the courts, law enforcement, jails, and hospitals to help improve the health of individuals and the quality of life in our community.

New Horizons is seeking a caring and compassionate SOAR Specialist in our Adult Case Management department to provide comprehensive direct support services which are structured to meet the needs of an adult population primarily composed of Serious Mental Illness and dual-diagnosed and/or referred persons who need assistance accessing benefits. As a SOAR Specialist, you will Assist with benefit applications, obtain financial information, cooperates with and advise staff, clients and families regarding benefit issues.


As a SOAR Specialist Your Responsibilities Will Include


  • Responsible for completing interviews with clients to gather information to complete SSI/SSDI applications

  • Ensures all SOAR Medical Summary Reports for client applications are complete.

  • Accompanies clients to appointments at the Social Security Administration and maintains communication with Social Security Administration (SSA) and Disability Determination (DD)

  • Responsible for reviewing cases with Local SOAR Lead. Attend scheduled staffing with SOAR lead and SEFBHN

  • Complete SOAR medical summary report for individuals application utilizing internal/external medical reports, obtain current psychiatric Dr, APRN or NHTC medical director signature for summary report.

  • Monitors progress of benefit process and provides assistance as required

  • Conducts case management activities for assigned clients to include assessment clients to include assessment of strengths and needs, service planning, service brokering, service and care monitoring, and client advocacy

Requirements


  • Associates degree from an accredited college or university and 3 years’ experience working with adults who have serious and persistent mental illness.

  • Bachelor's degree in a human service-related field with one (1) year of experience working with adults experiencing serious mental illness. Four (4) years experience may be considered in replacement of a degree

  • Certified

  • Valid and current Florida Driver's License required with no more than 6 points in the past 5 years
