Job summary

Spectraforce Technologies, Inc. is seeking a Transition Specialist to assist Northeast Ohio members with SSDI applications, requiring in-person connections with members in SNFs to collect information, obtain signatures, and travel to local SSA offices.

This 6‑month assignment offers hybrid work with regular travel to nursing facilities and SSA offices; training includes SOAR certification if needed, with standard Mon‑Fri hours and no holidays during training.

Qualifications

• 2–4 years of related experience in social work, nursing, long-term care, or documentation.
• SOAR training experience preferred but not required.

Responsibilities

• Review SNF census and gather medical records per SOP for SOAR SSA.
• Interview members and families to collect pertinent information.
• Communicate with members with language and speech difficulties.
• Navigate SNF environments and coordinate with social workers.
• Submit required SSA paperwork and follow up per feedback.
• Stay organized and detail-oriented in task execution.

Work Schedule

Mon-Fri 8a-5pm EST; 30 min lunch; No OT; No Holidays.

Training Schedule

Mon-Fri 8a-5pm EST - 1-2 weeks for SOAR cert (if the candidate doesn't already have SOAR cert) - additional 2 weeks of training - 1on1 - CAMERA ON - NO TIME OFF DURING TRAINING

Work Arrangement

Hybrid
Visiting members in nursing facilities & dropping off documents at local social security admin agencies.
About 2 trips per week, the rest will be remote documentation. Mileage is reimbursed
Not anticipated over 2 hour travel but depends on location

Position Summary

This position will be assisting members in northeast ohio with SSDI applications, which requires in person connections with members in SNFs to gather information via interviewing, obtaining signatures, and also trips to the local SSA office to drop off applications and paperwork.

Background & Context

This is a new position for growth to expand cost savings throughout the state, as NEO is our largest membership population. The team is diverse, respectful, supportive, and very connected although this position is highly autonomous. This role will be responsible for launching the expansion of SOAR across the state as these results will determine the future direction of expansion for this work.

Key Responsibilities

• review SNF census, navigate internal systems, gather medical records according to detailed SOP for SOAR SSA
• meet with member, interview member and family members to gather pertinent information
• communicate with members with language and speech difficulties
• navigate SNF environment and social workers for assistance
• understand SSA nuances, submit required paperwork and follow up timely according to feedback

Education / Certification

• Required: Bachelor's degree OR equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position.

Years of Experience Required

• Years of experience required: 2 - 4 years of related experience. Social work, nursing, skilled long term care facility, experience with documentation, member facing experience with written and verbal communication skills. ideally, some experience with SOAR applications but not required.
• Disqualifiers: inability to get SOAR certified, the department will provide the soar training and cert if necessary. Short term positions on resume (please state if contract and if completed), Looking for someone that is serious/committed about moving forward with the company.
