Job summary

Alignment Health is seeking a Supervisor, Care Coordination SNP to lead the SNP Care Coordination team and ensure timely health risk assessments, ICP development, and ICT delivery for SNP members with complex needs.

The role supports CM program compliance, oversees case management activities, and partners with CM leadership to drive program performance while maintaining confidentiality and regulatory compliance.

Qualifications

• 3+ years Care Coordination experience.
• 1+ years supervisory/lead experience in a health care setting.
• Knowledge of Managed Care Plans and medical terminology.
• Computer Skills: Microsoft Outlook, Excel, Word.

Responsibilities

• Create and maintain an environment that inspires and engages the team.
• Provide guidance to staff and manage complicated coordination requests from members, providers, or staff.
• Assist with development and oversight of SNP Care Coordination programs, SOPs and reports.
• Collaborate with other supervisors and managers to improve processes and workflow.
• Mentor, train, audit and coach Care Coordinators to ensure compliance with Alignment policies and regulatory requirements.
