Job summary

Alignment Health is seeking a Supervisor, Care Coordination SNP to lead the SNP care management team and ensure timely assessment, ICP development, and ICT delivery for members with complex needs.

Qualifications

• Minimum 3+ years Care Coordination experience.
• 1+ years supervisory/lead experience in a health care setting.
• Education/Licensure: High school diploma or GED; with related experience.
• Ability to analyze data for trends; knowledge of Managed Care Plans and medical terminology.
• Computer Skills: 25 WPM; Microsoft Outlook, Excel, Word.
• Read, write and speak English fluently; good interpersonal skills.

Responsibilities

• Create and maintain an environment that inspires growth and engagement of team members.
• Guide staff or manage coordination requests from members, providers, or staff.
• Assist with development and oversight of SNP Care Coordination programs, projects, and workflows.
• Develop SOPs and reports; review program performance for quality and timeliness.
• Collaborate with other supervisors and managers to improve processes and workflows.
• Mentor, train, audit, coach and manage non-clinical Care Coordinators to ensure policy compliance.
• Direct administrative work such as updating member records and case management tasks.
• Maintain confidentiality of information and ensure regulatory compliance.
• Review reporting to assign tasks and monitor time-sensitive items.
