Job summary

Enhance Rehabilitation is seeking a Speech Language Pathologist to join our team. You will evaluate and treat patients by following an individualized plan of care after an initial evaluation.

Responsibilities include administering standardized speech/language assessments, swallowing evaluations, and developing treatment plans while monitoring patient progress. Strong communication with patients, families, and the treatment team is essential in this role.

Qualifications

• Provide patient care in SNF setting following evaluation.
• Strong written, oral and interpersonal communication with patients, families, and the treatment team.

Responsibilities

• Demonstrate the knowledge and skills necessary to provide patient care appropriate in a SNF setting.
• Communicate effectively with patients, families, and other treatment team members when gathering and exchanging patient condition information.
• Administer standardized speech/language assessments, conduct swallowing evaluations, develop treatment plans and monitor progress.
