Job summary

Beach Gardens Rehab and Nursing Center in Far Rockaway, NY is seeking a dedicated social worker to support residents and families through transitions, including discharge planning and psychosocial care. You will complete assessments, participate in care planning, and advocate for residents’ rights and well-being.

The position is full-time, offering $60,000–$70,000 per year with health and dental insurance, paid time off and holidays, and a 401(k).

Qualifications

• Prior experience in a skilled nursing facility, including discharge planning.
• Bachelor’s or master’s degree in social work or related human services field.
• Familiarity with MDS, care planning, and Medicaid/Medicare.
• Warm, patient, and strong at communicating with residents, families, and staff.
• Organized and able to manage a busy caseload.
• LMSW/LCSW licensure welcome but not required.
• Bilingual is a bonus.

Responsibilities

• Plan safe discharges for short-term rehab patients, coordinating with families, home care agencies, DME vendors, and community services.
• Complete psychosocial assessments and assigned MDS sections on time.
• Take part in care plan meetings and interdisciplinary team meetings.
• Help new residents and families with admission and adjustment.
• Advocate for residents’ rights, preferences, and emotional well-being.
• Help residents and families with Medicaid applications, insurance questions, and community resources.
• Assist with advance directives, health care proxies, and MOLST conversations.
• Follow up on resident concerns and grievances.
• Keep documentation accurate, timely, and survey-ready.

Full-time | $60,000–$70,000 per year | Monday–Friday, 9-5

Do you love helping residents and families through big transitions? Beach Gardens is looking for a caring, organized social worker to join our team!

We’re a skilled nursing and rehabilitation facility caring for long-term residents and short-term rehab patients. Our social work team makes sure every resident feels supported, heard, and ready for what comes next, whether that’s going home or settling in with us.

What you’ll do:

• Plan safe discharges for short-term rehab patients, coordinating with families, home care agencies, DME vendors, and community services
