Job summary

Beach Gardens Rehab and Nursing Center in Far Rockaway, NY, is seeking a dedicated Social Worker to join our skilled nursing and rehabilitation team. The role focuses on discharge planning, psychosocial assessments, and coordinating with families to ensure smooth transitions for residents.

You will participate in care planning, support new residents, and advocate for residents’ rights and well-being. A background in a nursing facility and familiarity with Medicaid/Medicare are highly valued;

Qualifications

• Prior experience in a skilled nursing facility, including discharge planning (required).
• Familiarity with MDS, care planning, and Medicaid/Medicare is a big plus.
• Licensure (LMSW/LCSW) is welcome but not required.

Responsibilities

• Plan safe discharges for short-term rehab patients, coordinating with families and community services.
• Complete psychosocial assessments and assigned MDS sections on time.
• Take part in care plan and interdisciplinary team meetings.
• Assist new residents and families with admission and adjustment.
• Advocate for residents’ rights, preferences, and emotional well-being.
• Help residents with Medicaid applications and community resources.
• Assist with advance directives and MOLST conversations.
• Follow up on resident concerns and keep documentation timely and survey-ready.

Social Worker – Beach Gardens Rehab and Nursing Center, Far Rockaway, NY

Full-time | $60,000–$70,000 per year | Monday–Friday, 9-5

Do you love helping residents and families through big transitions? Beach Gardens is looking for a caring, organized social worker to join our team!

We’re a skilled nursing and rehabilitation facility caring for long-term residents and short-term rehab patients. Our social work team makes sure every resident feels supported, heard, and ready for what comes next, whether that’s going home or settling in with us.

What you’ll do:

• Plan safe discharges for short-term rehab patients, coordinating with families, home care agencies, DME vendors, and community services


• Complete psychosocial assessments and assigned MDS sections on time


• Take part in care plan meetings and interdisciplinary team meetings


• Help new residents and families with admission and adjustment


• Advocate for residents’ rights, preferences, and emotional well-being


• Help residents and families with Medicaid applications, insurance questions, and community resources


• Assist with advance directives, health care proxies, and MOLST conversations


• Follow up on resident concerns and grievances


• Keep documentation accurate, timely, and survey-ready

What we’re looking for:

• Prior experience in a skilled nursing facility, including discharge planning (required)


• Bachelor’s or master’s degree in social work or a related human services field preferred


• Familiarity with MDS, care planning, and Medicaid/Medicare is a big plus


• Warm, patient, and great at communicating with residents, families, and staff


• Organized and able to manage a busy caseload


• Licensure (LMSW/LCSW) is welcome but not required


• Bilingual is a bonus!
