Job summary

The Southeast Permanente Medical Group (TSPMG) seeks a physician licensed in Georgia to oversee SNF patient care for Kaiser Permanente members. The role involves on-site rounds, leading a NP and transitions team, and coordinating with SNF staff for discharge planning.

Responsibilities include goals of care discussions, timely documentation in KP HealthConnect, and collaboration with KP specialists to advance patient care in SNF settings.

Qualifications

• Licensed physician in the State of Georgia.
• Board Eligible graduates must become Board Certified within four years.
• Internist, Family Practitioner, Hospitalist, Geriatrician or Palliative Care Physician with SNF inpatient interest.

Responsibilities

• Round at SNFs at least twice weekly to address medical needs and care planning.
• Lead a team including a NP and KP Transition Case Manager to ensure timely care.
• Discuss goals of care and advanced directives with SNF staff and case manager.
• Attend meetings to discuss ongoing care planning and discharge planning.
