The Southeast Permanente Medical Group (TSPMG) in Atlanta, GA seeks a Skilled Nursing Facility Rounding Physician to oversee Kaiser Permanente members in SNF settings. You will lead a SNF rounding team, coordinate with an NP and a Transition Case Manager, and ensure appropriate levels of care across facilities.

Responsibilities include twice-weekly rounds, discharge planning, care-goal discussions, and close collaboration with SNF staff to optimize patient outcomes.

Qualifications

• Licensed physician in Georgia; Board eligible/new graduates must become board-certified within four years.
• Preferred: MD or DO with inpatient SNF experience.

Responsabilités

• Round Kaiser Permanente members at assigned SNFs at least twice weekly to address medical needs and care planning.
• Lead SNF team including an NP and KP Acute Transitions Case Manager to ensure appropriate care level and progress.
• Discuss goals of care and advanced directives with SNF staff and KP Transition Case Manager.
• Attend meetings with SNF staff and KP team to discuss ongoing care planning and discharge planning.
• Collaborate with KP specialists and outside physicians to coordinate care during SNF stay.
• Facilitate timely post-discharge follow-up with transitions team after SNF discharge.
• Coordinate with KP Quality Resource Management and other teams to meet regional goals.
• Maintain good relationships with on-site staff and transitions team; escalate system barriers when needed.
