Job summary

The Southeast Permanente Medical Group (TSPMG) seeks a part-time physician to join the Elder Care team in Georgia, focusing on outpatient care for Kaiser Permanente members 65 and older and those with Medicare needing special expertise of the team. Visits will be conducted face-to-face, virtually, or by telephonic means.

Qualifications

• MD/DO with residency in Internal Medicine or Family Medicine and board certification.
• 1 year post-residency experience caring for older patients in outpatient and/or facility settings.
• Strong communication with older patients, families/caregivers, and non-physician health professionals.
• Willingness to travel to various locations for patient care.

Responsibilities

• Conducts telephone, virtual and office visits.
• Assesses, treats, and/ or makes recommendations for care for geriatrics patients referred to Geriatrics.
• Discuss and document goals of care discussion and advance directives as needed.
• Develops medical aspects of care plan and coordinates tests, medications, referrals, and orders.
• Responds to MD Advice and Referrals in the in-basket and provides clinical feedback.
• Responds to e-consults from the PEEC clinic with pre-surgical recommendations.
• Collaborates with Palliative Care, Neurology, Neuropsychology, Behavioral Health, and Clinical Pharmacy.
• Supervises Geriatric NP, reviews notes, signs off on care plans and medications.
• Educates Primary Care and other specialists about geriatric care.
