Job summary

The American Academy of Family Physicians in Atlanta is seeking a licensed physician to manage care for members in a Skilled Nursing Facility (SNF). You will be part of a collaborative team that addresses medical needs, care planning, and transitions of care while documenting effectively.

The ideal candidate holds an MD or DO, is licensed in Georgia, and has experience in SNF or hospital settings. A competitive salary and benefits package is offered.

Qualifications

• Must be a licensed physician in Georgia.
• Board Certified within four years if Board Eligible.
• Internist, Family Practitioner, or similar with SNF management interest.

Responsibilities

• Round on KP members at assigned skilled nursing facilities.
• Lead a team in SNF including NP and Acute Transitions Case Manager.
• Collaborate with SNF staff for ongoing care planning.
• Establish good working relationships with KP providers.
• Participate in departmental meetings.
