Job summary

The Orchards Health Center is seeking a SNF Post Acute Case Manager to ensure the medical, emotional and social needs of residents are met in accordance with federal and state guidelines. This licensed social worker will oversee the SNF stay from admission through discharge, coordinating transitions and care plans.

Qualifications

• Requires Social Services Degree or LVN with 5+ years of experience.
• Excellent oral, written and interpersonal skills.
• Knowledge of Title 22/OBRA Regulations, Medicare, PPS, MDS, Care Planning and HIPAA.

Responsibilities

• Oversee SNF stay with focus on patient experience and transition planning for high-quality care.
• Coordinate care transitions and manage SNF stays for short- or long-term care.
• Develop social history and care plan implications including resident preferences.
• Assist residents and families with orientation to the facility and services.
• Meet regularly with patients and families to evaluate needs and readiness for transition.
• Collaborate with SNF staff, physicians and other disciplines on transition plans.
