Job summary

Enhance Rehabilitation in Wisconsin is seeking qualified Physical Therapists to join our team in a skilled nursing facility setting. You will evaluate and treat patients following an individualized plan of care and work with facility staff to ensure high-quality outcomes.

You will also supervise Physical Therapist Assistants, re-evaluate progress, document evaluations and progress notes, and educate patients on their injury, progress, and expected outcomes. A state license is required.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
