Enhance Rehabilitation in Wisconsin is seeking qualified Physical Therapists to join our team in SNF settings. You will evaluate and treat patients following individualized plans of care, and collaborate with facility staff to ensure high-quality outcomes.

Responsibilities include developing plans of care, setting measurable goals, documenting evaluations and progress, educating patients and families, and supervising Physical Therapist Assistants to maintain excellence in patient care.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
