Job summary

Enhance Rehabilitation in Wisconsin is seeking qualified Physical Therapists to join our team in skilled nursing facilities. You will evaluate and treat patients according to individualized plans of care and collaborate with facility staff to deliver high-quality services.

You will educate patients and families, set measurable goals, supervise Physical Therapist Assistants, re-evaluate progress, and maintain timely documentation of evaluations, progress notes, and discharges.

Qualifications

• Licensed Physical Therapist in good standing (state license).
• Experience in SNF setting preferred.
• Ability to develop and revise plans of care.
• Strong communication with patients and families.

Responsibilities

• Provide high-quality care to patients in SNF.
• Deliver excellent customer service to patients and families.
• Collaborate with facility staff to ensure optimal patient care.
• Evaluate new patients and set measurable goals.
• Treat patients and educate them on injury, progress and outcomes.
• Supervise Physical Therapist Assistants.
• Re-evaluate progress and adjust plans of care.
• Complete timely documentation including evaluations, re-evaluations, discharges and daily progress notes.
