Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our dynamic team. Based in a SNF setting, you will evaluate and treat patients following an individualized plan of care, collaborating with facility staff to ensure outstanding patient care and progress.

You will also supervise Physical Therapist Assistants and maintain thorough documentation of evaluations, re-evaluations, discharges and daily progress notes.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, addressing concerns and educating them on injury, progress and outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes
• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborating with facility staff to ensure excellent care for your patients
