Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join a dynamic SNF team. You will evaluate and treat patients, develop individualized plans of care, and work closely with facility staff to ensure the highest quality outcomes.

This role emphasizes patient education, progress documentation, and ongoing care plan adjustments. You will supervise Physical Therapist Assistants, re-evaluate progress, revise plans, and maintain timely and accurate records to support optimal patient

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
