Job summary

Enhance Rehabilitation is seeking an Occupational Therapist to join our team in a SNF setting. You will evaluate and treat patients following individualized plans of care, ensuring progress toward functional goals.

Responsibilities include patient and family education, re-evaluations, documentation, and supervising Occupational Therapy Assistants while collaborating with facility staff to maintain high-quality care.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes
• Supervising Occupational Therapy Assistants
• Collaborate with facility staff to ensure excellent care for your patients
• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
